The PMC Repair Process
1. Download a Repair Request Form.
2. Fill out all the information on the form.
3. Call Parks Medical at (509) 448-9112 and request an RMA (return merchandise authorization)
4. Be prepared to give the scope’s serial number and describe the problem.
5. Place the repair request form with the RMA Number and the scope in a shipping box.
